If you have a chronic condition, you need a way of having a true "medical home" including accessibility and the ability to help you track down needed resources. This could be a combination of a regular primary care doctor and a personal medical consultant, or a concierge primary care doctor. Either way, you'll need to buy services outside of your insurance premium. I know of no insurance plan out there right now providing people with chronic disease what they really need.
This is precisely why advice practices, health care advocates and clinical consultants are needed.
